Community Health Program

Allegheny Children’s Initiative’s Community Health services are designed to meet the integrated physical and behavioral needs of Allegheny County residents who receive services at Allegheny Children’s Initiative. Community Health Workers have a close understanding of the communities they serve and build trusting relationships with members, including those with diverse backgrounds, to connect them to resources and care. Community Health Workers coach and support individuals as they work toward their health care goals, including addressing their Social Determinants of Health needs.

Community Health Workers provide these services:Family piggy back ride

 

  • Assure that family members are connected to health care coverage.
  • Assist families in securing and maintaining stable and safe housing.
  • Connect families with healthy food resources.
  • Provide resources for utility assistance.
  • Secure safe and reliable transportation for families.

Eligibility Requirements:

 

Any family member of someone actively supported by Allegheny Children’s Initiative who needs assistance in connecting to health care coverage or addressing their Social Determinants of Health can be enrolled in Community Health services.
